You may also use this form to enter an unlisted Special or Discount
Offer that we may have emailed you.

The form may also be used to enter a Shipping Charge for orders
outside the United States and Canada. Example: The standard flat
shipping charge is $4.95 per order and is automatically added
during Shopping Cart Checkout. If we emailed you that your
shipping charge would be $12.00, you would enter "$7.05"
under "Price Each" ($12.00 - $4.95 = $7.05), "Shipping"
under "Stock Number and/or Description", and "1"
under "Quantity".
